Nutritional Therapy for Anorexia Nervosa & Bulimia Nervosa




Previously we saw through the symptomatology of the two most prevalent eating disorders amongst females anorexia nervosa and bulimia nervosa. Today we look at how we approach their treatment nutritionally. To begin with we need to understand what is the immediate call to action in rehabilitating an eating disorder patient. While counselling a patient it is important to understand the energy intake through the day by analyzing an average weekly diet recall.

Energy Intake - Be sure of it that the patient is extremely cautious of how many calories are consumed and what is their daily intake, therefore you must account for everything that they could be telling you. Carbohydrates - Malnourishment is the obvious effect of the eating disorders and although bulimia doesn't leave the person looking malnourished the constant purging leaves the gastrointestinal canal devoid of its good microorganisms and its tissue layer which secrets enzymes for digestion and also helps in absorbing the nutrients. By regular purging the cell layer sloughs off leaving the body malnourished as the nutrients do not get absorbed in the intestine. Due to the low carbs fixation common amongst anorexic patients the rehabilitation must account for lactic acidosis in the body, ketoacidosis, elevated serum amino acids and hypoglycemia. It should also be considered that due to constant starvation or binge eating the insulin secretion has been continuously tampered with and thus a very careful increase in the carbohydrate content should be taken. In patients with bulimia who undergo repeated purging could have esophageal bleeding which cannot allow roughage and hence low fiber carbohydrate should be included in the rehabilitation process. If fiber could be allowed then fruits rich in vitamins like E, C, and A should enrich the diet to reduce the oxidative stress developed int he body due to anaerobic respiration, mental stress, and substance abuse that occurs during reduction of body weight. So as to not set the insulin levels ballistic and also to provide as much energy in little food that gets ingested during initial stages, a frequent yet small quantity of simple carbohydrates rich in nutrients should be added such as dried figs, fruit pulp, sorbets, cottage cheese on small toasts, etc.

Protein - The muscle and tissue have been wasted to provide the body with some energy source and derive vitamins and minerals as they lack from the scanty food ingested. For binge - purgers there is a lot to deal with since they have corroded their teeth along with anorexics who have brittle fragile hair with scaly skin and fragile bones have a long way to sail till they rejuvenate their whole body with good protein that would heal, repair, build & strength the body frame to flesh all over. High biological value protein is the most essential in these cases as the intestinal aborption has been reduced and more over easily absorbed proteins would rehabilitate the body of its lost stores of essential amino acids, key building blocks in repair, immunity, strength. Proteins that like meat, veal, offal, seafood (shell & mollusks) even poultry and pulses should be given a wait for a while till the dairy, tofu, egg and fish (very little). In eating disorder s the kidney function does get affected as the body is dehydrated, filled with toxins, or on only protein diet. It becomes important to give protein in the diet based on the renal profile of the patient.

Fat - Well it will be a task to get a patient of an eating disorder to ingest fat in their diet but since we need to rehabilitate sparing the liver that has been over worked due to all the toxins, protein wasting and glucose deprivation, best suited fat could be with medium chain fatty acids or clarified butter, butter, dairy fat. Well one can also look at inherent essential fatty acids for immunity, repair and antioxidants by supplementing the diet with nuts and dried fruits as well as steamed or grilled fish. Fat in the diet will spare the proteins to be wasted and give the body lubrication, insulation, skin repair, protection against heart disease and gastric complications. Since the body is frail and so is the heart of the anorexic patients it would be advisable to stick to omega 3 rich fat source like vegetable oils.

Truth about Diet Patches, Pills, Powders & Belts

How many times do you search for latest diets for weight loss and find a new fad diet or pill or belt, or patch circling the web world especially those with celebrity names tagged on them? Every month or even every week there is a new development in fooling people globally that a certain pill or patch or diet can help you lose weight quickly. Some even take help of Ayurveda's name to sell themselves to the public. You however as a sane, intelligent audience should know the truth about these gimmicks as we know them.

There is another hormone called leptin secreted by white adipose tissue which is an indication of the amount of body fat its secretion is a mechanism by the body to tell the brain and gastric cells not to stimulate hunger as there is enough storage in the body. However the suppression of hunger by leptin is non-reactive in obese people.

When there are pills or patches that suppress hunger it is manipulating either leptin or ghrelin or other hormonal chain reaction that suppresses hunger. Although you must also know that people who fast or undergo fad diets get ghrelin surge and leads them to overeat faster. Diet Patch do have certain vitamins that are absorbed but their manipulation is also concentrated towards hunger and satiety. I AM Beautiful ....only if it is from within

I was watching an episode of GLEE this weekend because Mercedes Jones was going to sing one of my favorite songs "I am Beautiful" by Christina Aguilera. It was about the character Mercedes joining the cheerios and been asked to lose 10 pounds in a week since she was well "FAT" and that made her ugly. For those who aren't Glee watchers here is the story background which is why I am motivated to write about it. Mercedes starts going on a anorexic restrictive diet of water, maple syrup, cayenne pepper, lime, and Ipecac (vomiting drug) as suggested by her coach. Half way down the week she faints due to low blood pressure and just before that happens she sees all the kids in the cafeteria as food items. Now I don't know how many of you have had this experience either directly or indirectly but I have had people who had to be brain washed about what is the apt way to cut down weight and how much time is the right time to do it.

Many girls in pre teens and teens go through this heinous problem of starvation to cut fat out of their body and look thin well or try to. Unfortunately they succumb to these gimmicks of restrictive diets or fads or any short cut to their solution when they are most pressured or they need results by lets say end of the week. Seriously ? how long did it take them to put on that kind of weight or even those few extra pounds and expecting that the body would just lose it all because you want it that way is irresponsibly ridiculous. The healthiest way to come to your desired weight and shape is to eat right by a professional dietitian's advice only since they have the in depth know how and why how of your bodynomics and what you should be doing. Next step would be physical workout of minimum 30 min and maximum 45 - 60 min and take this one down if you think working out longer will help its a distant possibility to remain healthy. Your body needs only a certain amount of energy from the appropriate sources depending on your metabolic rate and to burn stored fat it can only be efficient within 1 hour of exercise anything more is sheer exhaustion (waste of time & effort).

Girls, you need to really understand that if you want your desirable shape and weight, you need to start from the inside out not other way round. If you don't do whats right for your body in the long run you will ruin all remaining gifts of life as a woman like looking blushingly beautiful. Your skin tone, hair, nails, bone structure can go for a toss if you do what Mercedes did in Glee and many girls like her. You will definitely end up older from the inside since your body depends on anaerobic respiration all the time due to lack of energy from food and therefore it starts ageing faster. It won't take it too long, much earlier than your 30's when you start looking saggy with your face and arms and nobody wants to be looking like a 50 year old when your just in your 20's. When losing weight make sure you do not lose more than 2 pounds a week and 10 pounds a month. Drink a lot of healthy fluids specially water and live a happy, beautiful life in your beautiful body. Wellness : Love yourself and avoid what Michael Jackson could have

Michael Jackson is one person who didn't deserve what he gave himself, disillusion, cynicism about his looks and his image, his health during his last years to be specific and more so his last days untill his last hour. For having adored his talent and genius to move the entire world with sheer energy of his music and his dance; I on a personal level am extremely disappointed by his death and professionally am motivated to responsibly educate who ever follows my blog or work about the harshest realities about not loving yourself for who you are. It has taken me almost a week to gather myself to write this article. I have been following his history through personal interviews, excerpts, documentary, making of his videos and the ongoing coverage post his passing by and one clear indication comes back to a single point troubled upbringing and low self esteem. We all are humans in the same right, having our weak moments in life one or more too often. Yes we all are equally vulnerable to criticism, ridicule, possible low confidence by self doubt and an unquenchable desire to be better than who we are in the present scenario - we are all that. But what we must remember in that weak moment while on verge to faulter that there is a bigger and valid reason why we are who we are. I was extremely lucky like many to be born healthy and to have grown into a presentable personality, although it would not be possible had it not been for the influx of positivity of my parents and family members all through my impressionable age. Like Michael's father once teased him about being ugly (to a boy who was the cutest and most handsome young man in his adolscence) because of his skin blemishes that made him cry and reclusive all the while, I too had one of my loved ones (not my family) say to my face that I should perhaps go for rhinoplasty. If you're already not aware of how good looking you are this comment can harm your menatality tremendously, it can lead you into extreme self doubt and contemplate procedures that you don't require to begin with. Of course I was deeply hurt and had my saddest moment there when I heard it but very soon the person was unceremoniously jutted out of my life for good. If god forbid I wasn't a strong individual and didnt have a family and a fleet of supportive friends can you imagine what that one statement could have ended me into ? I also thank my own intellect which is again a god given gift that I was sensible as a health professional to be responsible towards myself first.
Michael could have avoided all that lead him so far into his darkest moment had he refuted like his younger sister about such comments decades ago. We as his admirers could have had our Michael for a lot more years dancing to his music along with him if only he had loved himself for who he was just the way we didn't care about how he looked, it never mattered anyways. Infact I have over the years have guaged that people whether liked or disliked him unanimously agreed upon the fact that he was far better looking when hadn't gone under the knife. Wellness comes from within your soul, mind and into your body. Wellness needs positive energy, good food to run a healthy body, positive people surrounding you. Well its understandable that you can't always do much with who are surrounding you although your positivity will attract only positive people and that I have experienced. I have constantly written about the food that determines how our life behaves and yes food can also determine how we think as it influences neurotransimissions and has proven to have influence on mood regulation. When we dont eat the food that will allow us to think positively and love ourselves we automatically engage into a negative frame of mind and therby tending towards options like artificial parts of our body allowing us to be pseudo happy. Do you really think a perky or straight nose is the source of your happiness ? You ought to get a new dictionary for the word happiness in that case. I speak from my very traumatic experience of skin problems. I have a hyper reactive skin that leaves blemishes, scars, pigment with the change of climatic conditions, stress, physical interactions like holding an arm, clothing, carrying a heavy bag; I know its frivolous but true. I adore myself to no end all I need is my skin the way it originally has been so I need to take extreme care with its reactions. Being a averse to cosmetics its very difficult for me to slather sunscreen, disinfectant and non allergic products day in and day out all over my body but if thats what it takes to keep my skin healthy and safe I am all for hard work. Recently I panicked after having a therapy for my skin which had its temporary effects of marks but with immediate effect of 72 hours all was fine infact my skin was glowing and had done away with the blotchyness. Of course I have to take care with what I eat too but an external harm to your body can lead to procedures you wouldn't think of otherwise. Burn marks are one of them, please understand the reason plastic surgery is required to help people acquire a normal life out of their disformity not too become obessesively self destructive. The key to be happy is to primarily love what God or nature (who ever you believe in) created on this earth and that includes you. If you doubt your imperfect looks to get into a glamourous world let me shed some light on that aspect as well. There is no "glamour" once you're in the industry and again I speak from my experience. Its sheer perception that we in the glam world project out. On the contrary we have much hard work than anyone can imagine and I mean physical work. In the history of show business or any business a perfect nose has not raked in popularity and money and to prove it there can't be a bigger lesson than Michael Jackson. Love yourself to the fullest, share your joy wellness in its purest form will be where you are....
